Paul Coverdell National Acute Stroke Registry Performance Measures

  1. Deep Vein Thrombosis (DVT) Prophylaxis 
    Percent (%) of non-ambulatory ischemic stroke patients and hemorrhagic stroke patients who receive DVT prophylaxis by end of hospital day two.

  2. Discharged on Antithrombotic Therapy
    Percent (%) of ischemic stroke patients prescribed antithrombotic therapy at discharge.

  3. Patients with Atrial Fibrillation Receiving Anticoagulation Therapy
    Percent (%) of ischemic stroke patients with atrial fibrillation (paroxysmal or persistent) discharged on anticoagulation therapy.

  4. Thrombolytic Therapy Administered
    Percent (%) of ischemic stroke patients arriving within 2 hours of time last known well who receive IV tPA within three hours of time last known well.

  5. Antithrombotic Therapy by End of Hospital Day Two
    Percent (%) of ischemic stroke patients who receive antithrombotic therapy by the end of hospital day two.

  6. Discharged on Cholesterol Reducing Medication
    Percent (%) of ischemic stroke patients with LDL ≥100 or LDL not measured or on cholesterol reducer prior to admission who are discharged on cholesterol reducing therapy. 

  7. Dysphagia Screening 
    Percent (%) of ischemic stroke patients and hemorrhagic stroke patients who undergo a screen for dysphagia using a simple valid bedside testing protocol before receiving any food, fluids or medication by mouth.

  8. Stroke Education 
    Percent (%) of ischemic stroke patients and hemorrhagic stroke patients(or their caregivers) who received education or educational materials addressing all of the following:  risk factors for stroke, stroke prevention, warning signs for stroke, activation of emergency medical system, medications, and the need for continued medical attention.

  9. Smoking Cessation/Advice/Counseling 
    Percent (%) of ischemic stroke patients and hemorrhagic stroke patients with a history of smoking cigarettes (any cigarettes in past year) who receive smoking cessation advice or counseling at or prior to discharge.

  10. Assessed for Rehabilitation
    Percent (%) of ischemic stroke patients and hemorrhagic stroke patients who were assessed for rehabilitation services. 
    NOTE: All measures exclude those patients with an appropriate reason documented in the medical record for not receiving the therapy/intervention.
