| EnrollmentTo participate in the Minnesota Stroke Registry, hospitals must sign a grant agreement with the Minnesota Department of Health (MDH) that indicates a commitment to participate throughout the duration of the program. This grant award is provided to partially offset the cost of data collection for quality improvement. Steps: 1. Print, complete, and return the Participating Hospital Enrollment Form (see Downloads below) 2. MDH will send the grant agreement to your Program Contact to be signed by your authorized representative. 3. Your hospital returns the signed grant agreement to MDH. 4. MDH will send you a final signed agreement. This signifies your official enrollment into the program. You will receive information about orientation along with this agreement. SPECIAL NOTE: Hospitals participating in the American Heart Association's Get With The Guidelines-Stroke quality improvement program are encouraged to participate in the Minnesota Stroke Registry. Hospitals participating in both programs abstract and enter the same data elements only once (into the Patient Management Tool, provided by Outcome Sciences, Inc.).
